Today we can predict patients at risk of hospitalisation with upto 90% accuracy. This enables prioritisation of patients with most need in order to reduce hospitalisations and pressure on secondary care. 

This work has been done through ingest of the detailed clinical data and application of our AI trained models to this data to create a highly accurate prediction of patients that are at risk of hospitalisation in the next 6 months.

To make these predictions accessible to clinicians we also developed a state of the art AI Supported Analytics Product suite that enables deep patient insights to be provided to clinicians in support of their population health decision making
